Forms to Join AGEST

• Online member application

  It's quick and easy to join AGEST Super online.

• Member application

  A paper-based member application to join AGEST Super.

• Pension application form

  This application is for our standard and non-commutable pensions. It also includes a TFN declaration form. 

Forms for Making Contributions

• Co-contributions brochure

  A guide to this great Government initiative. The brochure also includes a contribution form.

• Member direct debit application

  A form to set-up a regular direct debit personal contribution to AGEST Super.

• Transfer your super form

  Combine all your other super into your AGEST Super account and save on multiple fees.

• Personal contributions form

  You should send this form to AGEST Super with your cheque when making a personal or spouse contribution.

Forms for Updating your Account Details

• Binding death benefit nomination form

  Form to nominate a binding benefit. Usually used in relation to death benefits." class="glossary">beneficiary or beneficiaries.

• Change of details form

  Use this form to update various account details, including: your tax file number; direct debit arrangements; changes or corrections to your contact details, name, date of birth, eligible service date or preferred beneficiaries and pension payments.

Forms for Insurance in AGEST

• Insurance Cover Transfer form

  Use this form to apply to transfer other insurance cover you have into AGEST.

• Insurance cover reduction form

  Form to request that your current level of AGEST Super insurance cover be reduced.

Forms for Withdrawing or Transferring from AGEST

• Proving your identity guide

  This guide provides information about AGEST's proof of identity requirements.

• Benefit application (pension)

  Pensioners can use this form to withdraw or rollover a lump sum from their pension account.

• Benefit application (cash withdrawal)

  Use this form to withdraw a lump sum from your account for payment directly to you.

• Benefit application (compassionate)

  For members who wish to access some or all of their AGEST Super benefit on compassionate grounds.

• Benefit application (hardship)

  For members who wish to apply to access their AGEST Super benefit on financial hardship grounds.

• Benefit application (rollover)

  Use this form to rollover a lump sum to another super fund. 

• Benefit application (temporary residents)

  Please note: This document has been replaced with a link to the Australian Taxation Office (ATO) website. The ATO have an online form for members who entered Australia on certain types of temporary residents visas and have now left Australia, and wish to claim the superannuation they accumulated in AGEST Super whilst in Australia.

• Certificate of Incapacity

  To be completed and sent to AGEST Super along with the 'Benefit Application' form if you are claiming a benefit because you have permanently retired early due to disablement or invalidity.
  
  If you have insurance cover in AGEST Super and wish to lodge a claim on it, you will also need to complete our insurer's claim forms.  Contact us on 1300 724 378 for a copy of the relevant form.

  Use this form to notify AGEST of the death of a member. 

  Your notification will allow AGEST to commence the appropriate claim process.

  Form to request that some or all of your personal contributions be treated as taxable contributions by AGEST Super.
