  • Third Party Authority Form

    Form to nominate a third party to make enquires about your account.

  • Binding death benefit nomination form

    Form to nominate a binding beneficiary or beneficiaries.

  • Change of details form

    Use this form to update various account details, including: your tax file number; direct debit arrangements; changes or corrections to your contact details, name, date of birth, eligible service date or preferred beneficiaries and pension payments.

  • Proving your identity guide

    This guide provides information about AGEST's proof of identity requirements.

  • Benefit application (pension)

    Pensioners can use this form to withdraw or rollover a lump sum from their pension account.

  • Benefit application (cash withdrawal)

    Use this form to withdraw a lump sum from your account for payment directly to you.

  • Benefit application (compassionate)

    For members who wish to access some or all of their AGEST Super benefit on compassionate grounds.

  • Benefit application (hardship)

    For members who wish to apply to access their AGEST Super benefit on financial hardship grounds.

  • Benefit application (rollover)

    Use this form to rollover a lump sum to another super fund. 

  • Benefit application (temporary residents)

    Please note: This document has been replaced with a link to the Australian Taxation Office (ATO) website. The ATO have an online form for members who entered Australia on certain types of temporary residents visas and have now left Australia, and wish to claim the superannuation they accumulated in AGEST Super whilst in Australia.

  • Certificate of Incapacity

    To be completed and sent to AGEST Super along with the 'Benefit Application' form if you are claiming a benefit because you have permanently retired early due to disablement or invalidity.

    If you have insurance cover in AGEST Super and wish to lodge a claim on it, you will also need to complete our insurer's claim forms.  Contact us on 1300 724 378 for a copy of the relevant form.

  • Death Benefit Notification form

    Use this form to notify AGEST of the death of a member. 

    Your notification will allow AGEST to commence the appropriate claim process.
